Colleagues must contact their Human Resources Department for policies and complete the required Authorization Form which must be filled out, signed by HR and required to be submitted upon check-in for Colleague & Friends/Family rates.  If signed form is not presented upon check in, the room rate will revert to the prevailing rate.
